Theoretically, fennel might increase levels of drugs metabolized by CYP3A4.

In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.

Activella combines two active hormones, estradiol and norethindrone, and both are processed by a liver enzyme called CYP3A4. Lab studies hint that fennel might slow this enzyme down a little. If that happened in the body, your hormone levels could drift a bit higher, which in theory could mean more of the usual estrogen-related effects like breast tenderness, bloating, nausea, headaches, or spotting.

Here's the important part: this is only seen in test-tube research, not in real people, so I wouldn't lose sleep over it. Eating fennel as a food or spice is very unlikely to cause any problem. Concentrated fennel supplements or the essential oil are where I'd be a touch more careful. If you plan to use those regularly, just check in with your pharmacist or doctor first.

Here's the short version: your Activella combines an estrogen (estradiol) with a progestin (norethindrone) to ease menopause symptoms like hot flashes and night sweats. Fennel contains plant compounds that act a little like estrogen in the body, so the concern is that large, concentrated fennel doses could theoretically compete with your estradiol at estrogen receptors and nudge how well your therapy works.

I want to be honest about the evidence here: this is purely theoretical, based on how fennel behaves in the lab, not on real cases in people. Cooking with fennel or enjoying it as a food almost certainly won't matter. If you're thinking about concentrated fennel capsules or the essential oil, just run it by your pharmacist or doctor first.
