Ospemifene falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fennel interacts with.

Theoretically, fennel might increase levels of drugs metabolized by CYP3A4.

In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.

Here's the situation with fennel and Osphena (ospemifene). Your body relies on a liver enzyme called CYP3A4 to clear ospemifene. Lab studies hint that fennel may slow that enzyme down. If it did, ospemifene could clear more slowly and build up a bit, which in theory might mean more estrogen-like effects or side effects such as hot flashes, vaginal discharge, muscle spasms, or sweating.

The important part: this is only a test-tube finding, and it has never been shown to happen in actual people. Everyday culinary fennel in cooking or tea is very unlikely to matter. Concentrated fennel supplements or the essential oil are where I'd be a little more cautious, so check with your pharmacist or doctor before adding those.

What Is Ospemifene Used For?

Brand name: Osphena

Osphena (ospemifene) is used to treat two specific symptoms of vulvar and vaginal atrophy — the thinning and drying of vaginal tissues that can happen after menopause. The first is moderate to severe dyspareunia (painful intercourse), and the second is moderate to severe vaginal dryness.

Both of these uses are for postmenopausal women only. Osphena is not used in children or in women who are pregnant or may become pregnant.
