Amiloride, Hydrochlorothiazide falls under DIURETIC DRUGS — the group Fo-ti interacts with.

Theoretically, fo-ti, particularly raw fo-ti root, might increase the risk of hypokalemia when taken with diuretic drugs.
Raw fo-ti root contains anthraquinone derivatives, which might have stimulant laxative effects and compound diuretic-induced potassium loss. In vitro research shows that fermented and processed fo-ti root have reduced laxative effects compared with raw fo-ti root.

Here's the thing about your water pill: the hydrochlorothiazide part can lower your body's potassium, while the amiloride is actually in there to help hold potassium in. That's a nice balance. Now, raw Fo-ti root contains anthraquinones that act like a stimulant laxative, and the worry is that loose stools could pull out potassium too, adding to whatever the diuretic is doing.

I'll be honest, this is a theoretical concern based on how these ingredients behave, not strong human proof. Because amiloride already protects potassium, big swings are less likely for you. Still, watch for muscle cramps, weakness, or an off heartbeat, and check with your pharmacist or doctor before adding Fo-ti. Processed Fo-ti has less laxative punch than the raw root.

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
