This combination pill pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic). Both are active drugs, not prodrugs, and your liver uses several enzymes, including CYP2C19, to help clear them. The concern here is that fo-ti may slow that CYP2C19 pathway down. If clearance slows, these medicines could build up a bit and their effects, good and bad, could feel stronger.

With more amitriptyline and perphenazine around, you might notice extra drowsiness, dry mouth, constipation, dizziness on standing, or a faster heartbeat. I'll be honest though: this is theoretical, seen only in lab and animal studies and never confirmed in people. Raw fo-ti seems to block the enzyme more than heavily processed extracts. Before mixing them, please run it by your pharmacist or doctor.

In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

This combination pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). Amitriptyline is partly handled by the liver enzyme CYP1A2, and lab tests suggest fo-ti may slow that enzyme down. If that happened in a person, amitriptyline could build up and you might feel its side effects more strongly: drowsiness, dry mouth, constipation, blurred vision, dizziness when standing, or a faster heartbeat.

Here's the honest part: this is only based on test-tube and animal work, and even those results were mixed (raw fo-ti slowed the enzyme, an alcohol extract sped it up). Nothing has been confirmed in humans. So I wouldn't panic, but I would loop in your pharmacist or doctor before pairing fo-ti with this medication, especially since fo-ti has its own liver-injury reports.
