CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Aspirin, Caffeine, Dover's Pwd falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Fo-ti interacts with.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product contains caffeine, which your body clears using an enzyme called CYP1A2. Fo-ti may affect that same enzyme, but the research is messy. In test-tube and animal studies, raw fo-ti seemed to slow CYP1A2, while an alcohol-based extract sped it up. Nothing like this has actually been confirmed in people.
If fo-ti did slow that enzyme, caffeine could linger longer, meaning more jitteriness, a racing heart, or trouble sleeping. Keep in mind this is theoretical only and based on lab data, so I wouldn't panic. The aspirin and Dover's Powder parts aren't really the concern here. Still, it's worth a quick chat with your pharmacist before pairing the two, especially given fo-ti's separate liver concerns.

In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Let me put this one in perspective. This combination product mixes aspirin, caffeine, and Dover's powder (which contains ipecac and opium). The concern on file is that fo-ti might slow down a liver enzyme called CYP3A4, which could let some medicines build up a bit. In theory, that could mean slightly stronger or longer-lasting effects from the caffeine or the opium portion, things like jitteriness, a racing heart, drowsiness, or constipation.
Here's the honest part: this is only a lab-dish idea. It has never actually been reported in people, and animal studies didn't even agree that fo-ti blocks this enzyme. So I wouldn't lose sleep over it, but since fo-ti has been tied to liver problems and this medicine carries opium, please run it by your pharmacist or doctor first.

ANTICOAGULANT/ANTIPLATELET DRUGS

This combination product contains aspirin, which thins your blood by making platelets less sticky. Fo-ti can add to that bleeding risk in two ways. First, the anthraquinones in fo-ti may have mild blood-thinning activity of their own. More importantly, processed fo-ti has been tied to rare cases of liver injury, and a damaged liver makes fewer clotting factors. In one reported case, a person's clotting time shot way up after months of fo-ti use. Stacked on top of aspirin, that could mean easier bruising, nosebleeds, or stomach bleeding.
This is based on anecdotal reports, not strong studies, so it isn't a sure thing. Still, please check with your pharmacist or doctor before combining them, especially if you notice dark stools or unusual bruising.
