Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2C9.

Here's a wrinkle worth knowing about losartan (Cozaar): it's actually a prodrug. Your body uses an enzyme called CYP2C9 to convert it into its more powerful active form, which does most of the blood-pressure-lowering work. Lab and animal studies hint that Fo-ti may slow down that very enzyme.

So if Fo-ti blocks CYP2C9, you'd expect less of the active form to be made, which could mean losartan works a little less well and your blood pressure isn't as well controlled. The good news: this is only theoretical and hasn't actually been seen in people, so I wouldn't lose sleep over it. Still, if you take both, it's smart to keep an eye on your blood pressure readings and check in with your pharmacist or doctor before mixing them.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's something interesting about losartan (Cozaar). It's actually a prodrug, meaning your body has to convert it into its active form to do most of its blood-pressure-lowering work. The enzyme CYP3A4 helps with part of that conversion. So if fo-ti slows that enzyme down, the theory is you might make a little less of the active form, which could make losartan work a bit less well at controlling your blood pressure.

That said, I want to keep this in perspective. This concern comes only from lab (test tube) studies, and even those disagree with animal research. It has never been reported in real people, and other enzymes also help activate losartan. So this is very much theoretical. Still, because fo-ti has been linked to liver injury, it's worth a quick chat with your pharmacist or doctor before combining them.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
