Piroxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2C9.
Animal and in vitro research suggests that fo-ti may inhibit CYP2C9. However, this interaction has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP2C9 to break down and clear piroxicam. Lab and animal studies hint that Fo-ti may slow that enzyme down. If that happened in people, piroxicam could linger longer and build up, which can mean stronger NSAID side effects. Think stomach upset or pain, heartburn, an increased chance of a bleed or ulcer, fluid retention, or strain on the kidneys.

The honest part: this is only a theoretical concern from test-tube and animal data, and it has not actually been reported in humans. Still, since both Fo-ti and piroxicam can be hard on the liver and gut, it's worth a quick chat with your pharmacist or doctor before combining them.

Piroxicam also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Fo-ti has been linked to cases of acute liver failure which can decrease clotting factor production and increase the effects of anticoagulants. In one case, a patient who had been stable on warfarin presented with acute hepatitis and an INR elevated to 14.98. The patient had been taking fo-ti for 90 days prior to admission. Discontinuation of warfarin and fo-ti lead to a decrease in the INR and full recovery. Theoretically, concomitant use of fo-ti with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients. Until more is known, monitor patients taking fo-ti and drugs that affect bleeding.
Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), dipyridamole (Persantine), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the concern with pairing Fo-ti and piroxicam (Feldene). Piroxicam is an NSAID, and on its own it can irritate the stomach lining and raise your risk of bleeding, especially in the gut. Fo-ti has been linked, in rare case reports, to liver injury. Since your liver makes the proteins that help your blood clot, a damaged liver plus an NSAID could add up to a higher bleeding risk.

This is based on anecdotal reports, not strong studies, so I don't want to overstate it. Still, watch for things like dark or bloody stools, unusual bruising, stomach pain, or yellowing of the skin or eyes. Please run this combination by your pharmacist or doctor before using both together.
