Tisotumab Vedotin-Tftv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the short version: Tivdak (tisotumab vedotin) is an antibody-drug conjugate. The antibody delivers a cancer-killing payload (called MMAE) into cells, and that payload is partly broken down by the liver enzyme CYP3A4. In a test-tube setting, Fo-ti has shown it might slow that same enzyme down. If the enzyme works slower, a bit more of the active drug could linger, which in theory might mean more of Tivdak's known side effects, like bleeding, nausea, fatigue, nerve tingling or numbness, and eye problems.

I want to be honest about how thin this evidence is. It comes only from lab studies, animal data actually disagrees, and it has never been seen in real people. Still, since Tivdak is serious chemotherapy, please loop in your oncology team or pharmacist before adding Fo-ti.

  • Tisotumab vedotin-tftv injection is used to treat cervical cancer (cancer that begins in the opening of the uterus [womb]) that has not improved or has come back after treatment with other medications or has spread to other parts of the body. Tisotumab vedotin-tftv is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
