PHENOBARBITAL (Luminal)
Folic acid can have direct convulsant activity in some people, reversing the effects of phenobarbital and worsening seizure control. Monitor closely for increased seizure activity.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
This combination product contains phenobarbital, which has anti-seizure (and sedative) effects. Here's the wrinkle with folic acid: in some people, higher doses of folate seem to push against phenobarbital's calming action on the brain. The concern is that this could weaken seizure control, meaning a person who was stable might be more likely to have a seizure.
This is based on limited clinical evidence, not strong proof, so don't panic. Everyday food folate and a standard prenatal or multivitamin amount usually aren't the issue. It's the larger, concentrated folic acid doses that are more likely to matter. If you take phenobarbital for seizures, please check with your pharmacist or doctor before adding folic acid so they can pick a safe dose and watch your seizure activity.
