Theoretically, ginkgo might decrease levels of drugs metabolized by CYP2C19.
Some clinical research shows that a specific ginkgo leaf extract (Remembrance, Herbs Product LTD) 140 mg twice daily can induce CYP2C19 enzymes and potentially decrease levels of drugs metabolized by these enzymes. However, other clinical research shows that taking ginkgo 120 mg twice daily for 12 days has no effect on levels of drugs metabolized by CYP2C19.

Small shifts can push you toward bleeding or toward clotting. The lab-based concern is that ginkgo might speed up a liver enzyme (CYP2C19) that helps clear warfarin, which could theoretically lower its blood-thinning effect over time.

Honestly, the evidence here is mixed and weak. One ginkgo product nudged that enzyme, while another study showed no effect. Here's the deal with warfarin and ginkgo. The interaction on file is about ginkgo possibly slowing down a liver enzyme called CYP1A2 that helps break warfarin down. In theory, if that enzyme is blocked, warfarin could build up and thin your blood more than intended. But honestly, the evidence here is weak and a bit mixed. Lab studies hint at it, while human studies suggest ginkgo may not really touch CYP1A2 much at all.

My bigger concern is separate: ginkgo itself can thin the blood. Stacked on warfarin (a narrow-margin drug), that could raise your bleeding risk, think easy bruising, nosebleeds, or bleeding gums. Theoretically, ginkgo might increase levels of drugs metabolized by CYP2C9.
In vitro, a specific standardized extract of ginkgo leaf (EGb 761) inhibits CYP2C9 activity . The terpenoid (ginkgolides) and flavonoid (quercetin, kaempferol, etc.) constituents seem to be responsible for this effect. Most ginkgo extracts contain some amount of these constituents. Therefore, other ginkgo leaf extracts might also inhibit the CYP2C9 enzyme. However, clinical research suggests that ginkgo might not have a significant effect on CYP2C9 in humans. Ginkgo does not seem to significantly affect the pharmacokinetics of CYP2C9 substrates diclofenac or tolbutamide.

Here's the situation with warfarin and ginkgo. Warfarin is a blood thinner that your liver clears using an enzyme called CYP2C9. In a test tube, ginkgo appears to slow that enzyme down, which in theory could let warfarin build up a bit and make your blood thinner than intended. That could show up as easier bruising, nosebleeds, bleeding gums, or blood in the urine or stool.

Honestly though, this lab finding hasn't held up well in actual human studies, so the real risk is uncertain. The bigger concern is that ginkgo itself can mildly thin the blood. Since warfarin is a drug where small swings matter a lot, please don't add ginkgo without checking with your doctor or pharmacist first, and keep up with your regular INR checks.

Theoretically, ginkgo might decrease levels of drugs metabolized by CYP3A4.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).

Here's the honest picture with warfarin and ginkgo. The enzyme angle (CYP3A4) is mostly theoretical and the data are conflicting, so I wouldn't lose sleep over it on its own. The bigger, more practical concern is that ginkgo can thin the blood on its own. Stack that on top of warfarin, which already lowers your blood's ability to clot, and you may tip toward easier bleeding.

Warfarin is a narrow therapeutic index drug, meaning small shifts matter a lot. Watch for unusual bruising, nosebleeds, bleeding gums, pink or red urine, or dark stools. Please don't start or stop anything yourself. Loop in your pharmacist or doctor first, and they may check your INR more closely if you add ginkgo.

Ginkgo has been shown to increase the risk of bleeding in some people when taken with warfarin. Theoretically, ginkgo might increase the risk of bleeding if used with other anticoagulant or antiplatelet drugs.
Several pharmacodynamic studies suggest that ginkgo inhibits platelet aggregation. It is thought that the ginkgo constituent, ginkgolide B, displaces platelet-activating factor (PAF) from its binding sites, decreasing blood coagulation. Several case reports have documented serious bleeding events in patients taking ginkgo. However, population and clinical studies have produced mixed results. Some evidence shows that short-term use of ginkgo leaf does not significantly reduce platelet aggregation and blood clotting. A study in healthy males who took a specific ginkgo leaf extract (EGb 761) 160 mg twice daily for 7 days found no change in prothrombin time. An analysis of a large medical record database suggests that ginkgo increases the risk of a bleeding adverse event by 38% when taken concurrently with warfarin. It has been suggested that ginkgo has to be taken for at least 2-3 weeks to have a significant effect on platelet aggregation. However, a meta-analysis of 18 studies using standardized ginkgo extracts, 80-480 mg daily for up to 32 weeks, did not find a significant effect on platelet aggregation, fibrinogen concentration, or PT/aPTT. In addition, a single dose of ginkgo plus clopidogrel or ticlopidine does not seem to significantly increase bleeding time or platelet aggregation. Also, taking ginkgo leaf extract daily for 8 days in conjunction with rivaroxaban does not affect anti-factor Xa activity; however, this study did not evaluate bleeding time.

Here's the deal with these two together. Warfarin already lowers your blood's ability to clot, and ginkgo may add to that by making your platelets (the cells that help plug a cut) less sticky. Stack them and you could bruise more easily, see nosebleeds or bleeding gums, or in rarer cases have a more serious bleed.

I'll be honest, the research is mixed. Some studies found no real change in clotting tests, but one large database review showed about a 38% higher chance of a bleeding event when ginkgo and warfarin were taken together. Because warfarin is a narrow-window drug where small shifts matter, don't start or stop ginkgo without checking with your pharmacist or doctor first, and keep up with your INR checks.

Ginkgo has been shown to increase the risk of bleeding in some people when taken with warfarin.
Several pharmacodynamic studies suggest that ginkgo inhibits platelet aggregation. It is thought that the ginkgo constituent, ginkgolide B, displaces platelet-activating factor (PAF) from its binding sites, decreasing blood coagulation. Information from a medical database suggests that when taken concurrently with warfarin, ginkgo increases the risk of a bleeding adverse event by 38%. There is also some evidence that ginkgo leaf extract can inhibit cytochrome P450 2C9, an enzyme that metabolizes warfarin. This could result in increased warfarin levels. However, population and clinical research has produced mixed results. Clinical research in healthy people suggests that ginkgo has no effect on INR, or the pharmacokinetics or pharmacodynamics of warfarin. A meta-analysis of 18 studies using standardized ginkgo extracts, 80 mg to 480 mg daily for up to 32 weeks, did not find a significant effect on platelet aggregation, fibrinogen concentration, or PT/aPTT. There is also some preliminary clinical research that suggests ginkgo might not significantly increase the effects of warfarin in patients that have a stable INR.

Here's the short version: combining ginkgo with warfarin may nudge your bleeding risk higher. Ginkgo can make your platelets a little less sticky, and one of its compounds may also slow down the liver enzyme (CYP2C9) that clears warfarin, which could let warfarin levels creep up. Since warfarin is a narrow therapeutic index drug, even small shifts can matter.

In real life that could show up as easier bruising, bleeding gums, nosebleeds, or blood in your urine or stool. That said, the evidence here is genuinely mixed. Several careful studies in stable patients found no real change in INR. So this is a possible concern, not a sure thing. Please don't start or stop either one on your own. Just give your pharmacist or doctor a heads up first.

Ginkgo is one of the world's most popular herbal supplements, mostly taken to support memory and circulation. The evidence for these uses is mixed and generally weak, and it is not proven to prevent memory loss or dementia. What Is Warfarin Used For?

Brand name: Warfarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
