Why this applies to Ketoconazole

Ketoconazole falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the simple version: glucomannan is a thick, gel-forming fiber, and when it sits in your stomach and gut alongside an oral medicine, it can physically slow or trap some of that drug so less gets absorbed. With oral ketoconazole, the worry is that you'd take in less of it. Since ketoconazole works by reaching high enough levels in your blood to fight off a fungal infection, lower absorption could mean it doesn't clear the infection as well as it should.

This is mostly a timing problem, not a reason to avoid one or the other. A practical fix many pharmacists suggest is spacing them out, taking your ketoconazole first and waiting about 30 to 60 minutes before your glucomannan. The direct evidence here is fairly limited, so please confirm the best schedule with your own pharmacist or doctor before changing anything.

What Is Ketoconazole Used For?

Brand name: Nizoral

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.

This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
