There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.

Here's the thing to know about combining gotu kola with oral ketoconazole: the concern is about your liver, not about gotu kola changing how the drug works. Ketoconazole taken by mouth is already one of the harder antifungals on the liver, which is exactly why doctors save it for situations where other options won't do. Gotu kola has been linked to liver irritation in a handful of case reports, so the worry is that stacking the two could add stress to the same organ.

To be fair, this is theoretical and the gotu kola reports are anecdotal, not proven. Still, I'd play it safe. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before pairing them.

What Is Ketoconazole Used For?

Brand name: Nizoral

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.

This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
