Why this applies to Trofinetide

Trofinetide falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the concern with pairing glucomannan and Trofinetide (Daybue). Glucomannan is a soluble fiber that turns into a thick gel in your stomach and gut. That gel can physically get in the way of how well an oral medicine is absorbed, meaning less of the drug may actually make it into your bloodstream. If that happens with trofinetide, you could get a weaker effect and less benefit for managing Rett syndrome symptoms.

The good news is this is easy to work around. The general advice is to take your oral medicine first, then wait about 30 to 60 minutes before taking glucomannan. This isn't proven specifically for trofinetide, so please run the timing by your pharmacist or doctor before combining them, especially since trofinetide is often given as a liquid.

What Is Trofinetide Used For?

Brand name: Daybue

Daybue and Daybue Stix are both approved to treat Rett syndrome in adults and pediatric patients 2 years of age and older. Rett syndrome is a rare neurological condition, and these are the branded forms of trofinetide available to treat it.

Both Daybue (oral solution) and Daybue Stix (powder that is dissolved to make an oral solution) carry this same indication. The safety and effectiveness in children under 2 years of age have not been established.
