Cannabidiol (cbd) and Trofinetide

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

Why this applies to Trofinetide

Trofinetide falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Cannabidiol (cbd) interacts with.

Theoretically, CYP3A4 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with these two. Trofinetide (Daybue) can act as a mild blocker of a liver enzyme called CYP3A4, and CBD happens to be broken down partly by that same enzyme. So the concern isn't that CBD changes your Trofinetide. It's the other way around: Trofinetide could slow how fast your body clears CBD, letting CBD levels drift a little higher than expected.

If that happens, you might notice more of CBD's usual effects, like drowsiness, tiredness, loose stools, or changes in appetite. I'll be honest with you though, this is a theoretical concern based on how the enzymes work, not something proven in studies. Still, since CBD product quality varies so much, it's worth telling your pharmacist or doctor you're using it so they can keep an eye on how you feel.
