Greater Celandine and Amiodarone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

HEPATOTOXIC DRUGS

Why this applies to Amiodarone

Amiodarone falls under HEPATOTOXIC DRUGS — the group Greater Celandine interacts with.

There is some concern that greater celandine can adversely affect the liver. Greater celandine has been linked to many cases of hepatotoxicity. Theoretically, concomitant use with other potentially hepatotoxic drugs might increase the risk of developing liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (TriCor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo).

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the issue with pairing these two: amiodarone is already one of the harder medications on your liver. It can raise liver enzymes and, in rare cases, cause real liver injury, which is why your doctor checks your labs periodically. Greater celandine has its own track record of harming the liver, with multiple reported cases of serious damage.

Stack two things that both stress the liver and you may raise the odds of trouble. Watch for clues like yellowing skin or eyes, dark urine, belly pain on the right side, nausea, or unusual tiredness. To be fair, this concern rests on anecdotal reports rather than strong studies, so it's a caution, not a certainty. Still, given celandine's spotty safety record, I'd talk with your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Amiodarone page

“Amiodarone (Pacerone, Cordarone) treats dangerous heart rhythms, and this is one supplement list I would genuinely be careful with. Of the 210 ingredients on file, 11 carry a major rating, and the everyday one is grapefruit, which blocks the liver enzyme that clears amiodarone and can push the drug's level higher than intended.”

About Greater Celandine

What is Greater Celandine?

Greater Celandine
  • Digestive and gallbladder complaints
  • Stomach cramps and bloating
  • Topical use for warts and skin growths
  • Liver and gallbladder 'cleansing'
  • Mild pain relief

Greater celandine is a traditional herb most often promoted for digestive and gallbladder complaints, but solid human evidence for any use is lacking. More importantly, it has been linked to serious and sometimes severe liver injury, so most experts advise against taking it by mouth. If you are considering it, talk with your pharmacist or doctor first.

About Amiodarone

What Is Amiodarone Used For?

Brand names: Pacerone, Cordarone

Amiodarone treats dangerous rhythm problems that start in the lower chambers of the heart: ventricular fibrillation and hemodynamically unstable ventricular tachycardia. In both, the heart beats so abnormally that blood pressure and blood flow can drop.

The tablets (including Pacerone) are for adults with documented, life-threatening, recurrent episodes who have not responded to other antiarrhythmics or cannot tolerate them. The IV injection (including Nexterone) is for starting treatment and preventing frequently recurring episodes in patients who are refractory to other therapy. Injection can also be used when someone needs oral amiodarone but cannot take medicine by mouth.
