Why this applies to Hydrocodone

Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Greater Celandine interacts with.

Research in vitro shows that chelidonine, a constituent of greater celandine, inhibits cytochrome P450 2D6 (CYP2D6) enzyme activity. Theoretically, greater celandine might increase levels of drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and many others.

Here's the wrinkle with hydrocodone: it's actually a bit of a prodrug. Your liver uses an enzyme called CYP2D6 to turn hydrocodone into hydromorphone, which is a much stronger pain reliever. Greater celandine contains a compound (chelidonine) that may block CYP2D6.

If that enzyme is slowed down, your body could make less of that potent active form, so your pain relief might feel weaker than expected. Keep in mind this is based only on lab studies, not real people, so it's theoretical for now. The bigger concern with greater celandine is its link to serious liver injury, which is why most experts advise against it. Please check with your pharmacist or doctor before combining the two.
