Tocilizumab falls under HEPATOTOXIC DRUGS — the group Greater Celandine interacts with.

There is some concern that greater celandine can adversely affect the liver. Greater celandine has been linked to many cases of hepatotoxicity. Theoretically, concomitant use with other potentially hepatotoxic drugs might increase the risk of developing liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (TriCor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo).

Here's the simple version: greater celandine has been tied to real cases of liver injury, and Tocilizumab (Actemra) can also raise liver enzymes and occasionally stress the liver. Putting two things together that both lean on the liver may add up, so the worry here is overlapping liver strain, not a change in how either one works.

Keep in mind this concern rests mostly on anecdotal reports, so it's a caution rather than a sure thing. Still, with Actemra your care team already checks liver labs periodically, and an unproven herb linked to liver damage isn't a smart partner. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please run any greater celandine plans past your pharmacist or doctor first.

Tocilizumab also falls under IMMUNOSUPPRESSANTS.

Preliminary clinical research suggests that taking a specific semi-synthetic derivative of the greater celandine constituent chelidonine (Ukrain; not available in North America) might stimulate immune responses in cancer patients. Theoretically, taking greater celandine might decrease the effects of immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the thinking behind this pairing. Tocilizumab (Actemra) works by calming down an overactive immune system to ease conditions like rheumatoid arthritis or giant cell arteritis. Greater celandine, on the other hand, contains alkaloids that some early research hints might nudge the immune system in the opposite direction, toward being more active. So in theory, the herb could work against what tocilizumab is trying to do and make your treatment less effective. That might show up as joint pain, stiffness, or swelling creeping back.

I want to be honest, though: this concern is mostly theoretical and based on a celandine derivative not even sold here, so we don't have real proof it happens with Actemra. The bigger worry with greater celandine is that it has caused serious liver injury. I'd skip it and check with your doctor or pharmacist first.

What Is Tocilizumab Injection Used For?

Brand name: Actemra

Tocilizumab injection is used to treat several serious inflammatory and immune-related conditions. Actemra (tocilizumab) is indicated for moderately to severely active rheumatoid arthritis in adults who haven't responded well to other disease-modifying drugs, giant cell arteritis in adults, systemic sclerosis-associated interstitial lung disease (SSc-ILD) to slow lung function decline, polyarticular and systemic juvenile idiopathic arthritis in patients 2 years and older, and severe or life-threatening cytokine release syndrome (CRS) caused by CAR T-cell therapy in adults and children 2 and older.

Actemra is also approved for hospitalized adults and children aged 2 and older with COVID-19 who are on corticosteroids and need supplemental oxygen or mechanical breathing support. Not every brand or route is approved for every use — your prescriber will choose the right form for your specific condition.
