Guar Gum and Flutamide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ORAL DRUGS

Why this applies to Flutamide

Flutamide falls under ORAL DRUGS — the group Guar Gum interacts with.

Guar gum might reduce the absorption of some oral drugs, potentially decreasing their effectiveness.
Clinical research shows that guar gum reduces or slows absorption of medications such as penicillin and metformin. To avoid changes in absorption, take guar gum 30-60 minutes after oral medications.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the practical concern with flutamide and guar gum. Because guar gum forms a thick gel in your stomach and gut, it can grab onto pills you swallow and slow or lessen how much actually gets absorbed. Flutamide is taken by mouth, so less of it reaching your bloodstream could mean a weaker effect against your prostate cancer. That matters, since flutamide is part of a carefully planned hormone treatment.

This is a possible interaction based on modest evidence (studies with other oral drugs), not something proven specifically with flutamide. A simple fix is timing: many pharmacists suggest taking guar gum 30 to 60 minutes after your medication. Please check with your own pharmacist or oncologist before adding any fiber supplement.

From our pharmacist’s overview on the Flutamide page

“A man taking flutamide (Eulexin) to block testosterone during prostate cancer treatment cannot afford big swings in the drug's level, and most of the 189 supplements on this page are capable of causing exactly that. The bulk of them work through the liver enzyme CYP3A4, which breaks flutamide down.”

About Guar Gum

What is Guar Gum?

Guar Gum
  • Constipation and bowel regularity
  • Diarrhea (as a stool bulking agent)
  • Lowering cholesterol
  • Blood sugar control
  • Feeling full for weight management

Guar gum is a soluble, gel-forming fiber from the guar bean that may modestly help with cholesterol, blood sugar, and bowel regularity. It is generally safe in food amounts, but concentrated supplements can cause gas, bloating, and a risk of throat or gut blockage if not taken with enough water. Always take it with plenty of fluid and check with your pharmacist or doctor first.

About Flutamide

What Is Flutamide Used For?

Brand names: Eulexin, Euflex

Eulexin (flutamide) capsules are used to treat prostate cancer in men. They are always used together with an LHRH agonist (a hormone-lowering injection), never alone.

Eulexin is indicated for two stages of prostate cancer: locally confined Stage B2-C prostate cancer (used alongside goserelin acetate and radiation therapy, starting eight weeks before radiation begins) and Stage D2 metastatic prostate cancer (cancer that has spread beyond the prostate, taken with an LHRH agonist until the disease progresses).
