Here's the thing to keep in mind: guar gum is a thick, gel-forming fiber, and when you swallow it that gel can wrap around a pill in your stomach and gut. That may slow down or lower how much of a medicine your body soaks up. Studies have shown this with drugs like metformin and penicillin, so it's reasonable to be cautious with an oral medicine like hydroxyurea (Hydrea, Droxia, Siklos) too.

Hydroxyurea is not a prodrug, so the worry here is simply getting less of it absorbed, which could mean weaker control of your sickle cell crises or your cancer. The easy fix is timing. A good rule is to take your hydroxyurea first, then wait 30 to 60 minutes before the guar gum. Please run this by your pharmacist or doctor before changing anything.

What Is Hydroxyurea Used For?

Brand names: Droxia, Hydrea, Siklos, Xromi

Hydroxyurea is used for several serious conditions. Droxia, Siklos, and Xromi are each indicated to reduce the frequency of painful crises and reduce the need for blood transfusions in patients with sickle cell anemia who have recurrent moderate to severe painful crises. Siklos is approved for adults and children 2 years and older; Xromi is approved for children 6 months and older; Droxia is approved for adults.

Hydrea capsules and Hydroxyurea capsules are indicated for resistant chronic myeloid leukemia and for locally advanced squamous cell carcinomas of the head and neck (excluding the lip) when used together with chemoradiation.
