Here's the simple version: guar gum is a thick, gel-forming fiber, and when it sits in your stomach and gut it can grab onto pills and slow or reduce how much medicine actually gets absorbed. With Kalydeco (ivacaftor), the worry is that less of your dose makes it into your bloodstream, which could mean it doesn't work as well at thinning that stubborn mucus and keeping your breathing and digestion steady.

Ivacaftor is not a prodrug, so this is about absorption, not activation. The good news is that the timing fix is easy. The general advice is to take guar gum about 30 to 60 minutes after your medication. Also remember ivacaftor should be taken with fatty food. Please run your routine by your CF team or pharmacist first.

What Is Ivacaftor Used For?

Brand name: Kalydeco

Kalydeco (ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 1 month and older who have at least one mutation in the CFTR gene that responds to ivacaftor. A specific genetic test is used to confirm whether a patient's mutation qualifies.

There is a long list of CFTR mutations for which Kalydeco is approved — including G551D, R117H, F508C, S549N, and many others. If your or your child's mutation is unknown, an FDA-cleared CF mutation test should be done first to confirm eligibility.
