Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the connection worth knowing about. Adrafinil turns into modafinil in your body, and modafinil can gently speed up a liver enzyme called CYP3A4. Your Kalydeco (ivacaftor) happens to be cleared by that very same enzyme. So in theory, pairing the two could push ivacaftor out of your system a little faster, leaving lower levels in your blood.

Why does that matter? Ivacaftor isn't a prodrug, it works as-is, so faster clearance could mean it works less well at keeping that thick mucus in check. The good news: this is only a theoretical concern based on how these drugs behave, not something proven in studies, and modafinil is a weak inducer. Still, since staying on a steady dose matters in cystic fibrosis, please check with your CF team or pharmacist before combining them.

What Is Ivacaftor Used For?

Brand name: Kalydeco

Kalydeco (ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 1 month and older who have at least one mutation in the CFTR gene that responds to ivacaftor. A specific genetic test is used to confirm whether a patient's mutation qualifies.

There is a long list of CFTR mutations for which Kalydeco is approved — including G551D, R117H, F508C, S549N, and many others. If your or your child's mutation is unknown, an FDA-cleared CF mutation test should be done first to confirm eligibility.
