Here's the gist with guar gum and Selinexor (Xpovio). Guar gum is a thick, gel-forming fiber, and when it sits in your stomach and gut it can grab onto pills and slow or lessen how much of them your body actually absorbs. Since Selinexor is taken by mouth and is not a prodrug (it already works as-is, not something your body has to switch on), the worry here is getting less of it into your system, which could mean it doesn't fight your cancer as well as it should.

This is based on modest evidence with other oral drugs, not Selinexor directly, so it's a reasonable caution rather than a sure thing. A simple fix is timing: separate them so guar gum comes well after your dose. Selinexor is a serious cancer medicine, so please run the exact spacing by your oncology team or pharmacist before combining them.

What Is Selinexor Used For?

Brand name: Xpovio

Xpovio (selinexor) is used to treat multiple myeloma in adults. When used together with bortezomib and dexamethasone, it is for adults who have had at least one prior treatment. This combination is sometimes called XVd.

Xpovio is also used with dexamethasone alone (called Xd) for adults with relapsed or refractory multiple myeloma — meaning the cancer came back or stopped responding — who have already tried at least four prior therapies and whose disease no longer responds to several other standard drug types.
