Here's the key thing with guar gum: it forms a thick gel in your gut that can trap or slow down medications you swallow, so less of the drug gets absorbed. For oral tretinoin (Vesanoid), which is the capsule form used to treat acute promyelocytic leukemia, that could mean you absorb less of your dose and get a weaker effect. With a cancer treatment, you really want full, reliable dosing, so this is worth taking seriously.

The evidence here comes from other oral drugs like metformin and penicillin, not tretinoin specifically, so it's a reasonable concern rather than a proven one. A simple fix is spacing them out: take guar gum about 30 to 60 minutes after your tretinoin. Please run the timing by your oncology team or pharmacist before combining them.

Tretinoin cream 0.02% is approved to help lessen the look of fine facial wrinkles in people who also follow a comprehensive skin care and sun-avoidance program — including daily sunscreen, protective clothing, and moisturizers.
