Muraglitazar falls under ANTIDIABETES DRUGS — the group Guava interacts with.

Theoretically, concomitant use with antidiabetes drugs might have additive effects and increase the risk of hypoglycemia. Animal research shows that guava leaf extract or guava fruit can have hypoglycemic effects. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, metformin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Muraglitazar is a blood-sugar-lowering medicine (it works on both PPAR pathways, similar in spirit to drugs like pioglitazone). Guava leaf and fruit, in animal and lab studies, have shown their own mild blood-sugar-lowering effect. So the concern here is simple: pile the two together and your glucose could dip lower than intended.

If that happens, you might feel shaky, sweaty, dizzy, hungry, or unusually tired, the classic signs of low blood sugar. I'll be honest though, this is mostly theoretical, based on early animal data, not strong human studies. Everyday guava as a fruit is unlikely to cause trouble; concentrated guava leaf supplements are the bigger question. If you'd like to try one, check your readings closely and loop in your pharmacist or doctor first.
