Metformin XR falls under ANTIDIABETES DRUGS — the group Guava interacts with.

Theoretically, concomitant use with antidiabetes drugs might have additive effects and increase the risk of hypoglycemia. Animal research shows that guava leaf extract or guava fruit can have hypoglycemic effects. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, metformin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the simple version: guava (especially the leaf extracts) may have a mild blood-sugar-lowering effect of its own. When you stack that on top of Metformin XR, which is already working to bring your glucose down, the two could add together and nudge your sugar lower than you intend.

I want to be honest about the evidence, though. This is mostly from lab and animal studies, so it's more of a theoretical caution than a proven problem. Eating a guava as a normal snack is unlikely to cause trouble. Concentrated guava leaf supplements are where I'd pay more attention. If you do try one, check your blood sugar a bit more often and watch for shakiness, sweating, or dizziness. Loop in your pharmacist or doctor before starting it so any dose tweaks can be made safely.
