Ertugliflozin falls under ANTIDIABETES DRUGS — the group Guava interacts with.

Theoretically, concomitant use with antidiabetes drugs might have additive effects and increase the risk of hypoglycemia. Animal research shows that guava leaf extract or guava fruit can have hypoglycemic effects. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, metformin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the simple version: guava (especially the leaf, in concentrated supplement form) may gently lower blood sugar on its own. Your ertugliflozin (Steglatro) is already working to push sugar out through your urine, so stacking the two together could nudge your numbers lower than expected.

Honestly, this concern comes mostly from lab and animal studies, so the evidence is on the weaker side, and ertugliflozin by itself rarely causes low blood sugar. The bigger worry is if you also take insulin or a sulfonylurea. Still, if you start a guava leaf product, keep an eye out for shakiness, sweating, dizziness, or confusion, and check your sugar more often at first. Eating the fruit as food is unlikely to be a problem. Loop in your pharmacist or doctor before adding it.
