Voxelotor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.

Here's the gist for Voxelotor (Oxbryta). Your body relies on a liver enzyme called CYP3A4 to break this medication down. Lab studies suggest the active compounds in guggul (guggulsterones) may speed that enzyme up. If that happens in people, your body could clear voxelotor faster, leaving less of it in your blood to do its job.

Voxelotor isn't a prodrug, so faster clearing simply means a weaker effect. For sickle cell disease, that could mean it works less well at helping your hemoglobin hold oxygen and keep red cells in shape. I'll be honest though: this is only a theoretical, test-tube finding, not proven in patients. Still, since this drug matters so much, please check with your pharmacist or doctor before combining the two.

Oxbryta (voxelotor) is approved to treat sickle cell disease (SCD) in adults and in children as young as 4 years old. It works by targeting the underlying problem in sickle cell disease — the abnormal hemoglobin that causes red blood cells to take on a rigid, sickle shape.

This approval was granted under the FDA's accelerated approval pathway, based on the drug's ability to raise hemoglobin levels. Continued approval may depend on results from confirmatory clinical trials showing broader clinical benefit.
