Empagliflozin falls under ANTIDIABETES DRUGS — the group Horsetail interacts with.

Theoretically, taking horsetail with antidiabetes drugs might increase the risk of hypoglycemia.
Equisetum myriochaetum has demonstrated hypoglycemic activity in clinical research. In an animal diabetic model, Equisetum giganteum had hypoglycemic effects. It is unclear whether other horsetail species have hypoglycemic effects.

Here's the simple version: empagliflozin (Jardiance) already lowers your blood sugar by flushing extra sugar out through your urine. Some horsetail species have shown a mild blood-sugar-lowering effect in lab and animal studies, so the concern is that stacking the two could push your sugar a little too low. If that happened, you might feel shaky, sweaty, dizzy, lightheaded, or unusually tired.

I want to be honest about how soft this evidence is. This is a theoretical worry based mostly on test-tube and animal research, not strong human studies, and we don't even know if all horsetail products act this way. Empagliflozin is not a prodrug, so this isn't about how it's broken down. Still, if you'd like to try horsetail, check with your pharmacist or doctor first and keep an eye on your blood sugar readings.

About Empagliflozin

What Is Empagliflozin Used For?

Brand name: Jardiance

Jardiance (empagliflozin) tablets are approved for several important uses in adults. It reduces the risk of cardiovascular death and hospitalization in adults with heart failure. It also slows the progression of chronic kidney disease by reducing the risk of serious kidney decline, end-stage kidney disease, cardiovascular death, and hospitalization. In adults with type 2 diabetes and established cardiovascular disease, Jardiance reduces the risk of cardiovascular death.

Jardiance is also used — alongside diet and exercise — to improve blood sugar control in type 2 diabetes in adults and children aged 10 and older. It is not recommended for blood sugar control in type 1 diabetes (it may raise the risk of a dangerous condition called diabetic ketoacidosis) or in type 2 diabetes patients with severely reduced kidney function.
