Why this applies to Hydrochlorothiazide

Hydrochlorothiazide falls under DIURETIC DRUGS — the group Horsetail interacts with.

Theoretically, taking horsetail with diuretic drugs might increase potassium loss and the risk of hypokalemia.
Laboratory research shows that various species of horsetail have diuretic properties. Due to its diuretic effects, there has been concern that taking horsetail along with potassium-depleting diuretics might increase the risk for hypokalemia. However, pharmacokinetic research in humans shows that taking horsetail 900 mg daily for 4 days does not affect urinary excretion of electrolytes, including potassium and sodium, despite having a diuretic effect similar to taking hydrochlorothiazide 25 mg daily. It is unclear if taking horsetail for a longer duration would affect electrolyte levels. Until more is known, use with caution.

Here's the concern with pairing horsetail and hydrochlorothiazide. Your water pill already pushes your kidneys to flush out salt and fluid, and along the way it can lower your potassium. Horsetail acts as a mild diuretic too, so the worry is that stacking the two could drop your potassium even lower (called hypokalemia), which can show up as muscle cramps, weakness, tiredness, or an irregular heartbeat.

The good news: this is mostly a theoretical concern. One human study found horsetail at 900 mg daily for four days didn't actually change potassium or sodium loss, even though it worked like a diuretic. We just don't know what longer use might do. To be safe, mention horsetail to your pharmacist or doctor so they can keep an eye on your potassium.

What Is Hydrochlorothiazide Used For?

Brand names: Esidrix, Hydro, Microzide, HydroDiuril, Hydro-D, Oretic, Aquazide H, Aquazide, HCTZ

Hydrochlorothiazide treats high blood pressure (hypertension) in adults and children, either alone or combined with other blood pressure medicines. Lowering blood pressure reduces the risk of strokes and heart attacks.

It also treats edema (fluid buildup) linked to congestive heart failure, hepatic cirrhosis (liver scarring), and kidney conditions including nephrotic syndrome. Hydrochlorothiazide tablets are additionally indicated for edema caused by corticosteroid or estrogen therapy, and edema from acute glomerulonephritis or chronic renal failure. Inzirqo (the oral suspension) is specifically approved for both hypertension and edema in adult and pediatric patients.
