Cyclophosphamide falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP2C19.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2C19 enzyme. This interaction has not been reported in humans.

Here's something worth knowing about this pairing. Cyclophosphamide is actually a prodrug, which means it doesn't do anything until your liver enzymes (including CYP2C19) convert it into its active, cancer-fighting form. Hu Zhang contains resveratrol, which in lab studies appears to block CYP2C19. In theory, slowing that enzyme could mean less of the drug gets activated, so you might get less of the benefit you're counting on from your treatment.

I want to be honest about how shaky this is though. This comes only from test-tube research, and it has never actually been seen in real people. Still, because cyclophosphamide is serious cancer therapy where getting the full effect matters, please don't add Hu Zhang without first checking with your oncologist or pharmacist.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the wrinkle with cyclophosphamide: it's actually a prodrug, meaning it starts out inactive and your liver enzymes (including CYP3A4) have to switch it on into its cancer-fighting form. Hu Zhang contains resveratrol, which in lab dishes can slow down CYP3A4. In theory, slowing that enzyme could shift how much active drug your body makes, so the way this interaction plays out isn't simple to predict.

The good news is that this concern is theoretical, based mostly on test-tube data, and one human study didn't see real-world problems. Still, cyclophosphamide is serious chemo where dosing is carefully calculated. Please don't add Hu Zhang without first clearing it with your oncologist or pharmacist, who know your exact regimen.

About Cyclophosphamide

What Is Cyclophosphamide Used For?

Brand names: Cytoxan, Neosar

Cyclophosphamide is used to treat many types of cancer in adults and children, including malignant lymphomas (such as Hodgkin's disease and Burkitt's lymphoma), multiple myeloma, several forms of leukemia, breast cancer, ovarian cancer, neuroblastoma, retinoblastoma, and advanced mycosis fungoides (a type of skin lymphoma). It is often given alongside other chemotherapy drugs rather than on its own.

Oral and injectable powder formulations (but not the Cyclophosphamide Injection solution concentrate) are also approved for minimal change nephrotic syndrome — a kidney condition — in children who have not responded well to, or cannot tolerate, steroid therapy. The injectable solution concentrate is approved for adult patients only due to its alcohol content, and is not approved for use in children.
