Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the gist with Vitrakvi (larotrectinib): your body relies on a liver enzyme called CYP3A4 to clear this medicine. Hu Zhang contains resveratrol, which in lab dishes can slow that same enzyme down. If that slowdown happened in real life, larotrectinib could build up a bit and you might notice more of its typical side effects, like dizziness, fatigue, nausea, constipation, or changes in liver blood tests. Larotrectinib works as-is (it is not a prodrug), so more in your system means stronger effects, not weaker ones.

The important part: this concern is theoretical, drawn from test-tube data, and one human study using a big resveratrol dose did not force any dose changes. Still, since larotrectinib is a serious cancer drug, please run any Hu Zhang use past your oncology team first.

About Larotrectinib

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration that can drive tumor growth. It is used when the tumor is metastatic (has spread) or when surgery to remove it would likely cause serious harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients must be selected for Vitrakvi therapy based on an FDA-approved test that confirms the NTRK gene fusion in their tumor. This approval is under the FDA's accelerated approval pathway, based on how many patients responded to treatment and how long those responses lasted. Continued approval may depend on results from follow-up clinical trials.
