Levobupivacaine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hu Zhang interacts with.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP1A2 enzyme. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Levobupivacaine (Chirocaine) is a local anesthetic, the kind of numbing medicine an anesthesiologist gives during surgery or for pain control. Your body partly clears it using a liver enzyme called CYP1A2. The resveratrol in Hu Zhang can slow that enzyme down in lab studies, so in theory it could let levobupivacaine build up a bit higher than expected.
I want to be honest about how soft this is: it comes from test-tube work, and nothing like it has actually been reported in people. If levels did climb, the concern would be the usual local anesthetic side effects (dizziness, a metallic taste, numb lips, ringing in the ears, or heart rhythm changes). Since this drug is given by your medical team, just tell them you take Hu Zhang before any procedure.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the gist for Levobupivacaine (Chirocaine). This is a local anesthetic, the kind your doctor or surgeon injects to numb an area for a procedure or to control pain. Your liver clears it partly using an enzyme called CYP3A4. Hu Zhang contains resveratrol, which in test-tube studies can slow down that same enzyme. In theory, that could let levobupivacaine linger a bit longer and build up, which might raise the chance of side effects like dizziness, a metallic taste, ringing in the ears, numbness around the mouth, or in rare cases heart rhythm changes.
That said, this is a theoretical concern based on lab data, and one human study using high-dose resveratrol didn't require any dose changes. Since levobupivacaine is usually given by a professional during a procedure, just mention any Hu Zhang use to them beforehand.
