Mavacamten falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP2C19.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2C19 enzyme. This interaction has not been reported in humans.

Here's the basic idea with Camzyos (mavacamten): your body relies on a liver enzyme called CYP2C19 to break it down and clear it out. The resveratrol in Hu Zhang may slow that enzyme down (this is seen in lab dishes, not yet in real people). If it does, mavacamten could clear more slowly and build up in your system.

Why that matters: mavacamten works by easing how hard the heart squeezes, so higher levels could push it too far, leading to a weaker pumping action (lower ejection fraction), shortness of breath, dizziness, or fluid buildup. This drug is dosed very carefully through a special monitoring program, so even small shifts count. Please don't start or stop anything on your own talk with your cardiologist or pharmacist first.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the concern with pairing these two. Mavacamten (Camzyos) is cleared from your body largely by a liver enzyme called CYP3A4. Hu Zhang is rich in resveratrol, and in test-tube studies resveratrol can slow that same enzyme down. If it slowed Mavacamten's breakdown, the drug could build up, and since Mavacamten works by weakening the heart's squeeze, too much can drop your heart function too far, leaving you short of breath, dizzy, swollen, or fatigued.

That said, this is theoretical. One human study using a high resveratrol dose didn't force any changes to CYP3A4 medications. Still, Mavacamten is a drug with very tight dosing and required monitoring, so please don't add Hu Zhang without first checking with your cardiologist or pharmacist.
