Severity Minor Likelihood Possible Evidence D

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: karaya gum is a bulky fiber that swells into a gel in your gut. When you swallow a pill like dapagliflozin (Farxiga) too close to that gel, the fiber can physically grab onto some of the medicine and slow or slightly reduce how much your body absorbs. This isn't about liver enzymes, and dapagliflozin is not a prodrug, so we're really just talking about absorption in the gut.

The good news is this is rated minor and only theoretical, so for most people it won't be a big deal. Still, if less dapagliflozin gets absorbed, it could work a little less well at controlling your blood sugar. An easy fix is timing: take your dapagliflozin first, then wait 30 to 60 minutes before the karaya gum. If you have questions, your pharmacist can help you set up a schedule.

About Karaya Gum

What is Karaya Gum?

Karaya Gum
  • Constipation (bulk-forming laxative)
  • Appetite and weight management (bulking agent)
  • Dental adhesive/denture fixative
  • Skin barrier for ostomy appliances
  • Food thickening and stabilizing

Karaya gum is a natural plant gum from the Sterculia tree that is mostly used as a bulk-forming laxative and as a thickener or adhesive in food, dental, and ostomy products. As a fiber-type laxative it may help with occasional constipation, but high-quality evidence is limited, and it should be taken with plenty of water to avoid choking or blockage. Talk to your pharmacist or doctor before using it, especially if you have swallowing problems or bowel conditions.
