L. casei preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. casei preparations by at least two hours.

Here's the thing to keep in mind with this combo: it's the probiotic that may lose out, not your medicine. Methenamine works by turning into formaldehyde in acidic urine, which acts like an antiseptic to keep urinary bacteria in check. Since it has germ-killing activity, taking it at the same time as your L. casei capsules could wipe out some of those live, friendly bacteria before they ever get to work.

This concern is mostly theoretical, based on how the two work rather than hard studies, so don't be alarmed. Your methenamine should still treat your urinary symptoms just fine. If you want full value from the probiotic, a simple fix is to space them out by at least two hours. Your pharmacist can help you set up a schedule.
