Larch Arabinogalactan and Epcoritamab-bysp

Here is the interaction we found between these two, with the evidence behind it.

IMMUNOSUPPRESSANTS

Why this applies to Epcoritamab-bysp

Epcoritamab-bysp falls under IMMUNOSUPPRESSANTS — the group Larch Arabinogalactan interacts with.

Theoretically, larch arabinogalactan might interfere with immunosuppression therapy due to immunostimulant effects. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and other drugs.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the quick picture with Epkinly (epcoritamab). This is a lab-made antibody treatment for certain lymphomas, and part of how it works actually involves revving up your immune system to attack cancer cells. Larch arabinogalactan is a fiber that some researchers think may gently stimulate immune activity. So on paper, the two could push in overlapping directions.

The honest truth is that this concern is theoretical, based on how each one behaves and not on real patient studies. We don't have solid evidence that the fiber changes how Epkinly works or worsens its side effects (like fever, low blood counts, infusion reactions, or the more serious cytokine release reactions your care team watches for). Because you're on a serious cancer therapy, please run any supplement past your oncologist or pharmacist before adding it.
