Here's the wrinkle with Apadaz: the benzhydrocodone part is a prodrug. Your body has to activate it (largely through CYP3A4) to release the active opioid that actually relieves pain. Lemongrass, thanks to a compound called citral, may slow down CYP3A4 — at least that's what animal and lab studies suggest. So in theory, less of the drug gets switched on, which could mean weaker pain relief than you expected.

The honest truth: this is theoretical and based only on early lab data, not human studies. A little lemongrass in your cooking or a cup of tea almost certainly won't matter. Concentrated capsules or the essential oil are the bigger question mark. If you lean on lemongrass supplements, just mention it to your pharmacist or doctor before changing anything.

Theoretically, lemongrass might increase the clearance and decrease the levels of glucuronidated drugs.
Animal research shows that lemongrass and its constituent citral induce uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation.

Here's the gist with Apadaz (benzhydrocodone with acetaminophen) and lemongrass. The acetaminophen part is cleared from your body partly through a process called glucuronidation. Lab and animal studies hint that lemongrass, and its lemony compound citral, may speed up the enzymes (UGTs) that handle this. In theory, that could clear the acetaminophen a bit faster and slightly lower its levels, which might make the pain relief feel weaker.

I want to keep this in perspective. This is based only on test tube and animal research, not on people, so it's quite theoretical. The lemongrass you'd taste in cooking or a cup of tea is very unlikely to cause trouble. Concentrated supplements or the essential oil are the bigger question mark. If you're using those regularly, just check with your pharmacist or doctor first.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
