Tenapanor Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lemongrass interacts with.

Theoretically, lemongrass might decrease the metabolism of CYP3A4 substrates.
Animal research shows that lemongrass and its constituent citral inhibit CYP3A4.

Here's the good news on this pairing: tenapanor (Ibsrela) is barely absorbed into your bloodstream. It works right inside the gut to treat constipation-predominant IBS, and very little of it actually gets into your body to be processed by liver enzymes like CYP3A4. So even though lemongrass might slow down that enzyme, it's unlikely to cause a meaningful buildup of tenapanor.

On top of that, the concern here is only theoretical, based on lab and animal studies rather than real people. The lemongrass in your cooking or a cup of tea is very unlikely to matter. If you're using concentrated lemongrass supplements or essential oil, just give your pharmacist a heads-up. The main thing to watch with tenapanor itself is diarrhea, so mention any worsening of that.

Tenapanor is approved for two distinct conditions, each under a separate brand. Ibsrela is used to treat irritable bowel syndrome with constipation (IBS-C) in adults. XPHOZAH is used to reduce high phosphorus levels (hyperphosphatemia) in adults with chronic kidney disease (CKD) who are on dialysis — specifically as an add-on when phosphate binders haven't worked well enough or aren't tolerated.
