Letrozole, Ribociclib Succinate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lime interacts with.

Animal research shows that phenolics in lime juice inhibit cytochrome P450 3A4. This has been demonstrated in rats treated with sildenafil, a CYP3A4 substrate, and high doses of lime juice.

Your Kisqali Femara combo (ribociclib plus letrozole) is cleared mainly by a liver enzyme called CYP3A4. There's some early lab and animal evidence that natural compounds in lime juice can slow that same enzyme down. In theory, if CYP3A4 is blocked, less of these drugs gets broken down, so a bit more could build up in your body. With ribociclib that might mean a higher chance of side effects like nausea, fatigue, low blood counts, or changes in your heart's rhythm (QT prolongation), and with letrozole, more hot flashes or joint aches.

The reassuring part: this is based on rat studies using high doses of lime juice, not people. A squeeze of lime on your food is very unlikely to matter. I'd just avoid large concentrated lime supplements and run any plans past your oncology pharmacist or doctor.
