Macitentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lime interacts with.

Animal research shows that phenolics in lime juice inhibit cytochrome P450 3A4. This has been demonstrated in rats treated with sildenafil, a CYP3A4 substrate, and high doses of lime juice.

Here's the gist: your body relies on an enzyme called CYP3A4 to clear macitentan (Opsumit). Lime contains plant compounds that, at least in animal studies, can slow that same enzyme down a bit. If macitentan isn't broken down as quickly, more of it could linger in your blood, which might nudge up its blood-pressure-lowering effects and side effects like headache, stuffy nose, low blood pressure, swelling in your legs, or feeling more tired or short of breath.

The reassuring part: this comes from rat research using high doses of lime juice, not people, and it's rated as a weaker, theoretical concern. Enjoying lime in your cooking or a drink is very unlikely to matter. If you're thinking about concentrated lime supplements or lime oil, just run it by your pharmacist or doctor first.

What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to lower the chance of being hospitalized because of PAH.

In the key clinical study, macitentan was used in adults with mainly moderate-to-severe PAH symptoms. Patients had PAH caused by unknown or inherited causes, connective tissue disorders, or congenital heart disease with repaired shunts.
