Concomitant use may increase anticholinergic effects and adverse effects. In a case report, a critically-ill male in his mid-60s developed a toxicological process attributed to anticholinergic syndrome following treatment with methylene blue for vasoplegia. Symptoms included anhidrosis, fever, lactic acidosis, mucosal dryness, and pupil dilation. Potential exacerbating factors included renal failure and use of fentanyl intra-operatively.

Here's the gist for this combo. The chlorpheniramine part of Ricobid is an older antihistamine that already carries anticholinergic, or drying, effects. Methylene blue can add to that same kind of activity, so stacking the two together may pile on those effects rather than cancel them out.

What that could look like is more of chlorpheniramine's usual side effects: a very dry mouth, blurry vision, dilated pupils, constipation, trouble urinating, feeling flushed or overheated, and drowsiness or confusion. Honestly though, this concern comes from a single hospital case in a very sick patient, so the everyday evidence is thin. Still, if you take methylene blue as a supplement, it's worth a quick chat with your pharmacist or doctor before pairing it with this product.

Combining serotonergic drugs with methylene blue can cause additive serotonergic effects, including serotonin syndrome. Methylene blue inhibits monoamine oxidase A. Intravenous or intrapulmonary use has resulted in serotonin syndrome, including reported fatalities, in patients taking some serotonergic drugs. This effect has not been reported with oral methylene blue. However, tell patients to avoid taking methylene blue with serotonergic drugs.

Here's the deal with Ricobid, which combines chlorpheniramine (an antihistamine) and phenylephrine (a decongestant). Methylene blue acts like a strong MAO inhibitor, meaning it blocks an enzyme that normally breaks down chemical messengers like serotonin and norepinephrine. Chlorpheniramine has mild serotonin activity, and phenylephrine raises norepinephrine, so pairing them with methylene blue could, in theory, let those signals build up too much.

What might that look like? Possible signs include a racing or pounding heart, higher blood pressure, agitation, sweating, shakiness, or feeling overheated. The serious cases (serotonin syndrome) have mostly been tied to injected methylene blue, not oral supplement forms, so real-world risk with a pill is likely lower. Still, please check with your pharmacist or doctor before combining them rather than guessing.
