Anticholinergic Drugs
Why this applies to Promethazine, Pseudoephedrine
Promethazine, Pseudoephedrine falls under Anticholinergic Drugs — the group Methylene Blue interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Phenergan-D, which pairs promethazine (an antihistamine with strong drying, anticholinergic activity) with pseudoephedrine (a decongestant). The promethazine part already blocks the same 'rest and digest' signals that methylene blue can also interfere with. Stacking the two may pile on those drying effects, so you could see more dry mouth, blurry vision, constipation, trouble urinating, flushing, fever, or feeling confused or drowsy.
I want to be honest about the evidence: this is based on a single case report in a very sick, hospitalized patient, so it's anecdotal, not a proven everyday risk. Still, since methylene blue as a supplement isn't well studied and can be potent, please run this combo by your pharmacist or doctor before mixing them.

From our pharmacist’s overview on the Promethazine, Pseudoephedrine page
“Phenergan D combines a sedating antihistamine, promethazine, with the decongestant stimulant pseudoephedrine, and its supplement list pulls hard in both directions, so this is one to treat with genuine care. The biggest cluster, from everyday caffeine sources like coffee and cola nut up to ephedra, stacks extra stimulation on the pseud…”
