The biggest cluster, from everyday caffeine sources like coffee and cola nut up to ephedra, stacks extra stimulation on the pseudoephedrine and can drive up blood pressure and heart strain. A second cluster, atropine-type plants such as henbane, deepens promethazine's drying, drowsy anticholinergic effects.
Activated charcoal earns a major rating for a different reason. It can soak up the medicine in the gut before it works, so keep the two well separated. A smaller group of ingredients can add to sun sensitivity while you take it.
