Here's the thing to keep in mind: Phenergan D contains pseudoephedrine, which is itself a stimulant decongestant. Black tea brings caffeine to the table, another mild stimulant. When you stack the two, the stimulant effects can simply add up. So you might notice a faster heartbeat, jitteriness, trouble sleeping, a touch higher blood pressure, or feeling restless or wired.

To be fair, this is based mostly on expert opinion rather than hard studies, and a single cup of tea is unlikely to cause problems for most people. The promethazine half is actually sedating, so effects can feel mixed. Still, if you tend to be caffeine-sensitive or have heart or blood pressure concerns, it's worth a quick chat with your pharmacist before pairing them.

Theoretically, phenothiazines might increase the levels and adverse effects of caffeine. Also, black tea may bind to phenothiazines and reduce their absorption.
Black tea contains caffeine. Phenothiazines can reduce the metabolism of caffeine by inhibiting cytochrome P450 1A2 (CYP1A2). Additionally, the tannins in black tea may bind to phenothiazines in the gastrointestinal tract and reduce their absorption.

Here's the gist with Phenergan D (which pairs the antihistamine promethazine with the decongestant pseudoephedrine) and your daily cup of black tea. There are two small, mostly theoretical concerns. First, promethazine can slow how your body clears caffeine, so the caffeine from tea may linger a bit longer. On top of the pseudoephedrine, which is already a stimulant, that could mean more jitteriness, a faster heartbeat, or trouble sleeping. Second, the tannins in tea may bind to promethazine in your gut and slightly reduce how much you absorb.

This rating is minor and based only on lab data, so a normal cup or two probably won't cause real trouble. If you notice extra jitters or restlessness, space the tea apart from your dose and check with your pharmacist.
