Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the deal with this pairing. The interaction on file is really about your thyroid extract, not about Phenergan D itself. Thyroid hormone is fussy about absorption, so when you take it too close to food or to another pill, its uptake can drop a bit and the thyroid dose may work less well.

The good news is that promethazine and pseudoephedrine (the cold and allergy combo in Phenergan D) don't have a strong, well-known clash with thyroid hormone here. This is rated minor. To keep your thyroid product working its best, just space it out, ideally take it on an empty stomach and wait 30 to 60 minutes before other medicines.
