Here's the thing to keep in mind: cilostazol (Pletal) is already an antiplatelet medicine, meaning it makes your platelets less sticky so blood flows more easily through narrowed leg vessels. Nattokinase works on clotting too, by helping break down fibrin and thinning the blood a bit. Stack the two together and you're essentially adding one blood thinner on top of another, which can tilt you toward easier bruising, nosebleeds, bleeding gums, or in rare cases something more serious.

I'll be honest that the evidence here is on the weaker side, mostly lab studies plus a single case report of brain bleeding when nattokinase was added to aspirin. Still, since bleeding risk is the concern, please check with your pharmacist or doctor before combining them rather than starting nattokinase on your own.
