Here's the thing to know about pairing nattokinase with esmolol. Esmolol is a fast-acting beta-blocker given through an IV in hospital settings, usually to control a racing heart rate or high blood pressure during procedures. It already works to lower blood pressure. Nattokinase, the enzyme from fermented natto, may gently nudge blood pressure down too. Stack the two together and, in theory, your blood pressure could dip lower than expected.

I'll be straight with you: this concern is theoretical, based on how each works rather than real cases of people running into trouble. Since esmolol is carefully dosed and monitored by your care team, just make sure they know about any supplement you take so they can watch for low blood pressure, dizziness, or a slow heartbeat.
