Labetalol falls under ANTIHYPERTENSIVE DRUGS — the group Niacin interacts with.
The vasodilating effects of niacin can cause hypotension. Furthermore, some clinical evidence suggests that a one-hour infusion of niacin can reduce systolic, diastolic, and mean blood pressure in hypertensive patients. This effect is not observed in normotensive patients.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Labetalol already lowers your blood pressure by relaxing blood vessels and slowing your heart rate. High-dose niacin can widen blood vessels too, so stacking the two together may push your pressure down a bit more than expected. This isn't about niacin changing how your body breaks down labetalol, it's just two things nudging blood pressure in the same direction.
What you might notice is dizziness, lightheadedness, feeling faint when you stand up, or that warm flushing niacin is known for. The evidence here is fairly mild and more of a theoretical caution, and ordinary food amounts of B3 are very unlikely to matter. It's the concentrated, drug-strength niacin doses worth flagging. Before adding one, check in with your pharmacist or doctor.

Niacin has been associated with cases of liver toxicity, especially when used in pharmacologic doses. Sustained-release niacin preparations appear to be associated with a higher risk of hepatotoxicity than immediate-release niacin.

Here's the connection worth knowing. Both labetalol and high-dose niacin can, on their own, occasionally stress your liver. Labetalol has been linked to rare liver issues, and niacin in big, drug-like doses can do the same, especially the sustained-release kind. So the worry is simply that using both together might add up and make liver irritation a little more likely.
To be upfront, this is a theoretical concern based on how each drug behaves, not on strong proof that the combo causes harm. The vitamin amounts in food or a regular daily multivitamin are very unlikely to matter here. If you take concentrated niacin alongside labetalol, just mention it to your pharmacist or doctor. They may check your liver with a simple blood test and watch for signs like yellowing skin, dark urine, or unusual fatigue.

They can be used alone or alongside other blood pressure medicines, such as diuretics. By lowering blood pressure, labetalol helps reduce the risk of serious events like stroke and heart attack.
Labetalol injection products (LABETALOL HCl, Labetalol HCl in Dextrose, Labetalol HCl in Sodium Chloride, Labetalol Hydrochloride intravenous) are used specifically for severe hypertension and are given intravenously in a hospital setting.
