Acetaminophen, Phenylephrine, Chlorpheniramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Nutmeg interacts with.
Animal research suggests that intraperitoneal injections of myristicin, a constituent of nutmeg, can induce CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo cold tablet. The concern on file is that nutmeg may speed up a liver enzyme called CYP1A2, which could lower blood levels of medicines that enzyme handles. If that happened, you'd theoretically get a bit less effect, not more, so your fever and aches relief from the acetaminophen, or the runny-nose help from the chlorpheniramine, might feel slightly weaker. None of these are prodrugs, so this isn't about a dangerous buildup.
Honestly though, this is based only on lab and animal research using injected nutmeg compounds, not real people sprinkling nutmeg on food. Normal cooking amounts almost certainly won't matter. If you're tempted by large medicinal nutmeg doses, skip them and check with your pharmacist first.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your Super Cold Tabs: the ingredient that matters here is the chlorpheniramine, an antihistamine that works partly by blocking acetylcholine in the body. Lab and animal studies hint that nutmeg may raise acetylcholine levels, which could work against chlorpheniramine and slightly blunt how well it tames a runny nose or sneezing. In theory, the two could each take a little edge off the other.
Honestly, this is a very weak, theoretical concern based only on animal data, and the sprinkle of nutmeg you'd use in cooking is unlikely to do anything at all. I wouldn't lose sleep over it. Just steer clear of large "medicinal" nutmeg doses, which can be genuinely toxic, and check with your pharmacist if you have questions.
