Nutmeg and Chlorpromazine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Nutmeg interacts with whole categories of medication. Chlorpromazine falls into 2 of those categories, so it can interact with Nutmeg in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CNS DEPRESSANTS

1 of 2

Why this applies to Chlorpromazine

Chlorpromazine falls under CNS DEPRESSANTS — the group Nutmeg interacts with.

Theoretically, nutmeg might increase the risk of additive sedation when taken with CNS depressants.
Animal studies suggest that nutmeg extracts and several volatile oils in nutmeg, such as methyleugenol, isoeugenol, safrole, myristicin, trimyristin, 1,8-cineole, and geranyl acetate, have sedative effects. One animal study shows that petroleum ether extracts of nutmeg can potentiate the effects of pentobarbital or phenobarbital. However, evidence from other animal research suggests that the nutmeg constituent myristicin can actually reduce sleeping time in rats pretreated with phenobarbital.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist with nutmeg and chlorpromazine (Thorazine): chlorpromazine is already a pretty sedating medication, and some compounds in nutmeg have shown calming, sleep-promoting effects in lab and animal studies. The theory is that stacking the two could add up to extra drowsiness, dizziness, low blood pressure, or feeling foggy and slowed down.

I want to be honest, though, this is theoretical and only based on animal research, with mixed findings, so it isn't well proven in people. The sprinkle of nutmeg you'd use in baking or on eggnog is very unlikely to matter. The real concern is large "medicinal" doses of nutmeg, which are dangerous on their own. To be safe, run any nutmeg supplement past your pharmacist or doctor first.

ANTICHOLINERGIC DRUGS

2 of 2

Chlorpromazine also falls under ANTICHOLINERGIC DRUGS.

Theoretically, concomitant use of nutmeg and anticholinergic drugs might decrease the effectiveness of either agent.
Animal research suggests that nutmeg extract can inhibit acetylcholinesterase and might increase acetylcholine levels.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea: chlorpromazine has some anticholinergic activity, meaning part of how it works (and part of why it causes side effects like dry mouth, constipation, blurry vision, and drowsiness) involves dialing down a brain and body chemical called acetylcholine. Lab and animal studies hint that nutmeg may push acetylcholine levels up by blocking the enzyme that breaks it down. In theory, that could tug against chlorpromazine's anticholinergic effects and slightly blunt them.

I want to be honest with you, though: this is theoretical and based only on animal data, not real-world studies in people. Normal cooking amounts of nutmeg almost certainly won't matter. Large 'medicinal' doses of nutmeg are a different story and can be toxic on their own. If you're using concentrated nutmeg supplements, just run it by your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

About Nutmeg

What is Nutmeg?

Nutmeg
  • Digestive upset
  • Diarrhea
  • Sleep and relaxation
  • Pain and inflammation
  • Flavoring spice

Nutmeg is a popular cooking spice that has long been used in traditional medicine for digestion and other complaints, but there is little solid human research to support its medicinal use. In normal cooking amounts it is generally safe, but larger "medicinal" doses can cause serious poisoning, so it should never be taken in high amounts.

About Chlorpromazine

What Is Chlorpromazine Used For?

Brand names: Thorazine, Thorazine Spansules

Chlorpromazine is used to manage psychotic disorders and treat schizophrenia. It also controls nausea and vomiting, relieves restlessness and apprehension before surgery, and controls the manic phase of manic-depressive illness. Other uses include relief of intractable hiccups, treatment of acute intermittent porphyria, and use as an adjunct in tetanus treatment.

In children ages 1 to 12, it is used for severe behavioral problems marked by explosive or combative behavior, and for short-term treatment of hyperactive children with conduct disorders. The oral forms are also approved for the management of manifestations of psychotic disorders broadly, in addition to the uses shared with the injection form.
